Please be aware that your Anthem Blue Cross policy will cancel effective 12/31/17.  You will need  to apply for new coverage by the  12/15/17 deadline in order to have coverage for a 1/1/18 effective date.

**December 15th is a VERY CRUCIAL deadline date!!!

under employer group options.

MAJOR IMPORTANT FACTORS 

to choosing the right plan will be the provider network.   Please have a conversation with your doctor's office to confirm which individual carrier networks they will be contracted with for 2018.

  • Blue Shield will be ONLY PPO in the 2018 market
